How to schedule an appointment:  
We are scheduling appointments (no walk-ins) through the following link:

Before arrival at the clinic: 
1. Bring a copy of your insurance card. All insurances are accepted.  
2. Bring your COVID Vaccine Card for the staff to update.

Who is eligible: 
We are providing the first, second or Booster dose of the Pfizer COVID vaccine. (It is the same vaccine for all three doses.) 

The booster doses of the Pfizer COVID-19 vaccine are for the following individuals who have completed the 2-dose Pfizer Covid Vaccine series at least six months ago: 

  • People 65 years old
  • People aged 50–64 years with underlying medical conditions should receive a booster shot
  • People aged 18–49 years with underlying medical conditions may receive a booster shot, based on their individual benefits and risks
  • People aged 18-64 years who are at increased risk for COVID-19 exposure and transmission because of occupational or institutional settings, including healthcare workers, may receive a booster shot, based on their individual benefits and risks

The underlying medical conditions are: 

  • Cancer
  • Chronic kidney disease
  • Chronic lung diseases, including COPD (chronic obstructive pulmonary disease), asthma (moderatetosevere), interstitial lung disease, cystic fibrosis and pulmonary hypertension 
  • Dementia or other neurological conditions
  • Diabetes (Type 1 or Type 2)
  • Down Syndrome
  • Heart conditions (such as heart failure, coronary artery disease, cardiomyopathy or high blood pressure) 
  • HIV infection
  • Immunocompromised state (weakened immune system)
  • Liver disease
  • Overweight (BMI greater than 25 and under 30) and obesity (BMI of 30 and above) 
  • Pregnancy
  • Sickle cell disease or thalassemia
  • Smoking, current or former
  • Solid organ or blood stemcell transplant
  • Stroke or cerebrovascular disease, which affects blood flow to the brain
  • Substance use disorders

People who received the Moderna or Johnson & Johnson vaccine are not yet eligible for the booster shots authorized by the federal government. Once Moderna or Johnson & Johnson boosters are authorized, we will provide them as well.
